Academic Foundation Department (AFD) of The University of Cambodia (UC) would like to inform students, who had enrolled with AFD in Promotion 9 Term 2 Academic Year 2010-2011, that AFD is processing its work to issue the Foundation Year Certificates for those who will complete the Foundation Year Courses successfully at the end of this term. Students are badly requested to get the Clearance Form and fill it out all required information at AFD and submit it back to AFD on January 30, 2012 at 5pm.
Requirements:
1- Verify your personal information with Bac II Certificate (or other official equivalence)
2- Write your name clearly in Latin word
3- Attach with 2 photos, size 4Í6 (new photo less than 6 months).
AFD is not responsible for late submission or failing to submit the form back and any misinformation you have made by yourself for issuing the AFD certificate.
